You can now fill out your Accident Report online and print out copies for your files and for CFSA.
To access the form you'll need Adobe Acrobat Reader Version 7* on your computer and, ideally, a high speed Internet connection. (If you have a dial-up connection, be patient, it will just take a minute or two longer for the form to appear.) If you don't have Adobe Acrobat at all or if you need to upgrade, you can download Version 7 for free from the Adobe Web site.
*To confirm your version, open your Adobe Reader program, click on "Help," then "About Adobe Reader." The version is listed at the top left of your computer screen.
When you've finished filling out the form and you're satisfied with the information you've provided, simply click on "Print" at the bottom of the form. For now, you cannot SUBMIT the form to CFSA electronically ~ but we're working on it and will let you know when it's possible to do so. Until then, please continue to either mail or FAX your completed reports to us. You'll find our address on the form.
To access the form, all it takes is a click of your mouse: General Liability Accident Report.
You'll also find a copy of the Accident Report in your fair's Red Book (Claims and Loss Reporting Guide) under General Liability, Tab 2, that you can print out and use.
Want to save a copy of the completed online form?
If you have a full version of the Adobe Acrobat software program (not just Acrobat Reader)
on your computer, you can save a copy of the completed Accident Report. To ensure that
the report opens with your Acrobat program (so that you are able to save it), you’ll need to check your Acrobat “Preferences” settings. Here's how:

Open Adobe Acrobat.
Click on “Edit” in the main menu bar and then click on “Preferences” in the “Edit” menu.

In the “Preferences” window, click on “Internet” in the left hand “Categories” column. If the boxes for “Display PDF in browser”, “Allow fast Web view”, and “Allow speculative downloading in the background” aren’t checked, do so by clicking each box with
your mouse.

Click “OK” to close the “Preferences” window.

Close Acrobat. The next time you open the report PDF from CFSA’s Web site, it will automatically
open in Adobe Acrobat and you can save the report just as you would save any other
PDF document.

Why is it important for CFSA General Liability Pool members to fill out an Accident Report?
Reporting
losses promptly to CFSA is essential to efficient claims handling. So send
your accident reports to us right away and resist the temptation
to batch them together until the end of your fair. We know everyone's
busy during fairtime, but delays can prove costly to you, your fellow
pool members, and to us as evidence and witnesses may be hard to
locate, and claimants may become more and more upset as time goes
on.
Report Serious Injuries Immediately
Serious injuries should be reported ASAP by phone to Rosalyn Johnson,
(916) 263-6171, so an investigation can be launched. (After normal
business hours, call [916] 921-2213 for further instructions.) We recommend
that you keep a camera on hand to document the accident scene.
Hand drawn diagrams with measurements are valuable too.
